Cobra is a much less critical part of the healthcare system now that the ACA and it's various protections for things like pre-existing conditions exists. Prior to the ACA if there had not been COBRA someone with cancer who could no longer work would have simply died, or gone broke paying out of pocket, because no private insurer would have taken on a current cancer patient. Thankfully today it's not the only option.
