The drug I am on is tirzepatide + B12.

Many people my age have SOME arthritis, which involves inflammation. I don’t see it in my hands very much…maybe at little bit at times… but I have occasional bouts of gout which is a form of arthritis. I would not say gout
is “normal “but I consider some arthritic joints to be a normal part of aging.

You’re right that most drugs have a side effect and often several side effects, including I suppose depletion of essential nutrients.

That's exactly what my wife is taking. As she explained it to me the dosage is graduated over time so her first 4 injections were at a certain dosage and her current (5th) injection and the following 6th, 7th and 8th injections are at a higher dosage level. I'm not sure what happens after that although she tells me that like you, she's considering eventual micro doses to help maintain the success she's had.

Is B12 depleted when weight loss drugs are taken? I am too chicken to use any pharmaceuticals but if it works for you, that's great.

I do not know if B12 is depleted. From my understanding, EJ Lily owns the patent to tirzepatide and it cannot be sold in pure form by anyone else. So, compounding pharmacies sell this drug with a B12 addition thereby operating within legalities.

My doctor said she goes up in dosage when weight loss stops for a month. That seems a bit draconian to me, but I haven’t reached that yet so we shall see.
